One of the best sites we found was "Estate Planning for Non-Human Family Members" by Gerry W. Beyer, Professor of Law, St. Mary's University School of Law. 

The site is comprehensive, covering a variety of issues related to the topic.  In addition, it provides a listing of state statuyes and also provides sample provisions to be used in documenting your wishes where your pet(s) are concerned.
